Return to Lender Request Form - You have the right to request your funds be returned to the lender up to 14 days after the date of disbursement notice, if you have not cashed the refund check. Once the 14 days have passed or after you have deposited funds into your personal bank account you must contact your servicer and make payment arrangements with them.
Enrollment & Graduation Verification Request Form - Your loan servicer(s)* may want verification of your deferment/enrollment status and/or graduation date. The Registrar & Student Services Office completes this verification for you. You must complete the request form and submit it to the Registrar & Student Services Office.
